Let’s break down the common reasons why a heart’s electrical axis might shift, and then we’ll understand these in practical terms, not textbook language.

First, enlarged heart chambers. Think about this logically—if one side of the heart gets bigger, there’s more muscle mass on that side. Electrical flow follows muscle, so the axis shifts toward the bigger chamber. This happens with chronic high blood pressure, heart failure, or intense athletic training.

Second, heart attacks. When part of the heart muscle dies, it doesn’t conduct electricity anymore. The electrical flow has to go around that dead tissue, which can shift the axis. This is why sometimes a 12-lead can show evidence of an old heart attack even if the patient doesn’t remember having one—the electrical pattern changed.

Third, chronic lung disease. When someone has COPD and their lungs are overinflated, it changes how the heart sits in the chest and puts strain on the right side of the heart. That strain can cause the right heart to enlarge, shifting the axis.

Fourth, blocked electrical pathways—we’ll talk more about this in a moment, but basically, if electricity can’t flow through its normal route, it finds an alternate path, which changes the direction.
